Session Fees & Insurance

Session Fees & Insurance

Insurance
I am an in-network provider with several major insurance carriers ie Medicare, Anthem, BlueCross/BlueShield of Georgia, Aetna, United Healthcare, TriCare, Humana, PHCS. However please check with your insurance company to determine if I am in-network provider with your insurance carrier as well as your co-pay or co-insurance amount prior to our session.

Many insurance plans have the option of “out-of-network” benefits which significantly reimburse policyholders for services. I am happy to provide you with the paperwork needed to seek out-of-network reimbursement. Psychotherapy services are usually eligible expenses for reimbursement from medical flex-spending accounts.

I also offer a sliding scale fee based on income and discounts for long term weekly therapy if private pay.

Payment
Payment is expected at the time of service unless other arrangements have been made with me. I offer several methods of payment. Please contact the office to inquire.

Good Faith Estimate
Beginning January 1, 2022, federal laws regulating client care have been updated to include the “No Surprises” Act. Under the law, healthcare providers need to give clients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services called a “Good Faith Estimate” (GFE) explaining how much your medical care will cost.

This new regulation is designed to provide transparency to clients regarding their expected medical expenses and to protect them from surprises when they receive their medical bills. It allows clients to understand how much their health care will cost before they receive services.

- The Good Faith Estimate shows the costs of items and services that are reasonably expected for your health care needs for an item or service.

- The Good Faith Estimate is not a contract and does not obligate you to obtain any services from the provider(s) listed, nor does it include any services rendered to you that are not identified here.

- The estimate is based on information known at the time the estimate was created. The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. There may be additional items or services your provider may recommend as part of your care that must be scheduled or requested separately and are not reflected in this Good Faith Estimate. You could be charged more if complications or special circumstances occur. If this happens, federal law allows you to dispute (appeal) the bill.

- You have the right to initiate a dispute resolution process if the actual amount charged to you substantially exceeds the
estimated charges stated in your Good Faith Estimate (which means $400 or more beyond the estimated charges). You
may contact the health care provider or facility listed to let them know the billed charges are higher than the Good Faith Estimate. You can ask them to update the bill to match the Good Faith Estimate, ask to negotiate the bill, or ask if there is
financial assistance available.

- You may also start a dispute resolution process with the U.S. Department of Health and Human Services (HHS). If you choose to use the dispute resolution process, you must start the dispute process within 120 calendar days (about 4 months) of the date on the original bill. There is a $25 fee to use the dispute process. If the agency reviewing your dispute agrees with you, you will have to pay the price on this Good Faith Estimate. If the agency disagrees with you and agrees with the health care provider or facility, you will have to pay the higher amount.
For questions or more information about your right to a Good Faith Estimate or the dispute resolution process, visit https://www.cms.gov/nosurprises/consumers or call 1- 800-985-3059.

The initiation of the client-provider dispute resolution process will not adversely affect the quality of the services furnished to you.

There are a number of factors that make it challenging to provide an estimate on how long it will take for a client to complete therapeutic treatment, and much depends on the individual client and their goals in seeking therapy. Some clients are satisfied with a reduction in symptoms while others continue longer because it feels beneficial to do so. Others begin to schedule less frequently and may continue to come in for “tune ups” or when issues arise. Ultimately, as the client, it is your decision when to stop therapy.
At Elysium Consulting, we must provide a diagnosis for all clients for both ethical, legal, and insurance reasons - as well as required by the "No Surprises Act". A formal diagnosis occurs after an assessment has been completed. That will take place 1-5 sessions after beginning psychotherapy. If you choose to decline a formal diagnosis, we will not update the GFE.
